Fractional CO2 Laser Technology and Our Clinical Applications: Advanced Skin Resurfacing at the Tissue Level

Dr. Melik Candar October 28, 2026 7 Min Read

In medical aesthetics and dermatology practice, Carbon Dioxide (CO2) laser systems are the most effective method for deep tissue resurfacing, pore tightening, and scar (wound/acne scar) repair. In our clinic, instead of old-generation glass tube devices whose energy stability drops over time, we use a high-tech metal tube 10600 nm CO2 laser device that works with millisecond precision and produces continuous, stable laser energy with a homogeneous beam in every shot.

Working Principle: Deep Ablation and Fractional Micro-Tunnel Architecture

The carbon dioxide laser operates at a wavelength of 10600 nm with an ablative character. The ablation mechanism here creates controlled and aggressive tissue destruction that can reach the deep layers of the dermis by instantaneously vaporizing the cellular water in the target tissue. Unlike superficial peeling procedures, it reaches the deep layers of the skin, triggering biological damage and subsequent powerful neocollagenesis (new collagen production).

The fractional working principle of the device combines this aggressive power with millimetric control:

  • Fractional Scanning: Laser energy divides the skin into mm² areas and focuses on micro-points perfectly and equally spaced from each other according to tissue density.
  • Micro Tunnels (Microthermal Zones): The device opens vertical micro tunnels into the deep tissue according to the determined energy aperture time (pulse duration at the nanosecond level).
  • Rapid Epithelialization: Thanks to the intact tissue islands left around the micro-tunnels, the skin's capacity to repair and renew itself is maximized; the healing process is shortened, and the risk of thermal damage is eliminated.

Our device offers three basic protocol handpieces according to clinical needs:

  1. Fractional Handpiece: Deep skin resurfacing, pore tightening, laser peeling, and scar revision.
  2. Surgical (Cautery) Handpiece: Bloodless surgical excision of skin lesions at the micron level.
  3. Vaginal and Genital Handpieces: Special anatomical handpieces used for tissue tightening in gynecology and genital aesthetics.

Treatment Protocols and Application Areas in Our Clinic

1. Skin Resurfacing, Laser Peeling, and Pore Tightening
The vertical micro-channels opened by the fractional handpiece fundamentally narrow the enlarged pore structure that has lost its elasticity over time. While the photo-damaged upper layer of the skin is removed with controlled ablation, new and healthy fibroblast activity is initiated in the lower layers. This effect restores the skin's lost firmness, smooth texture, and clear radiance.

2. Advanced Protocol in Scar and Acne Scar Treatment: Subcision + Fractional CO2 Laser
Under deep acne scars and atrophic scars, there are fibrous bands (subcutaneous adhesions) that pull the skin inward and create depressions. Laser shots applied only from the surface may be insufficient to break this retraction. Therefore, we combine the Subcision method in our clinic:
• Subcision Stage: With the help of a special blunt-tipped (non-tissue damaging) needle called a cannula, these hard fibrous adhesions under the skin are mechanically separated from the scar tissue above. The released tissue rises to its natural level.
• Fractional Laser Stage: Intense collagen induction is provided with fractional micro-tunnels opened over the released area; the depression fills from the base, giving the skin surface a homogeneous form.

3. Treatment of Surgical and Cesarean Scars
We apply the fractional CO2 laser very successfully not only on the facial area but also in the treatment of cesarean scars and other surgical incision scars that cause aesthetic anxiety on the body. The powerful ablative effect of the laser remodels the thickened (hypertrophic) wound tissue, allowing the scar to soften, the raised tissue to flatten, and the wound color to approach the surrounding skin tone.

4. Bloodless Skin Lesion Treatment (Cautery Handpiece)
The surgical cautery handpiece of our device vaporizes skin lesions such as verruca (warts), simple papillomas (skin tags), seborrheic keratosis, and xanthelasma by coagulating them at the micron level. Because it simultaneously seals the vessels, the procedure is completely bloodless; it requires no stitches, healthy tissue is preserved, and the risk of scarring is almost non-existent.

5. Genital and Gynecological Uses
The gynecological handpieces included in the device's hardware can also be safely used in medical and aesthetic genital area applications in line with specific needs.

Who is This Treatment Not Suitable For?

Because the Fractional CO2 laser is a very powerful and deep tissue resurfacing procedure, we do not apply this procedure in some cases to ensure your safety. The procedure is only applied to individuals over 18 years of age. Fractional CO2 laser is not performed on people with the following conditions:

  • Pregnant Women: It is not applied to individuals during pregnancy.
  • Melasma and Rosacea Patients: It is not suitable for those with intense melasma (deep pigmentation/dark spots) and active rosacea. (Please note: CO2 laser is not a method used to "erase" deep skin spots; on the contrary, it can trigger such spots with heat.)
  • Those with Skin Cancer or Suspected Skin Cancer: It is not applied to people with active skin cancer or suspicious precancerous lesions in the application area.
  • Unhealed Wounds: It is not applied to people whose surgical wound has not completely healed, or who have open wounds or active infections in the area.
  • Roaccutane Use: Those who have used systemic isotretinoin (Roaccutane, Zoretanin, and similar acne medications) within the last 6 months.
  • Keloid Tendency and Herpes: Patients with a history of developing abnormal raised scars (keloids) and those with active herpes simplex infection in the application area are not suitable candidates for this treatment.

Clinical Experience, Session Protocol, and Healing Process

Before the application, a local anesthetic cream is applied to the area and left for about 30 minutes. Despite the ablative power of the device, thanks to this comfort provided, the sensation of pain remains at a medium-low level. Treatment parameters are precisely calibrated according to the patient's skin phototype, the depth of the scar, and their social life plan. In some cases, a single, powerful session is sufficient, while in some indications, controlled session intervals are preferred.

The healing process after the procedure requires sensitive discipline:

  • 5-7 Days of Crusting: Punctate, micro-crusts form over the micro-tunnels opened by the laser. These crusts are a natural barrier that protects the underlying tissue from the external environment; they should absolutely not be peeled off, they must shed in their own course.
  • Photoprotection: To prevent the fresh epithelial tissue from developing any hyperpigmentation (darkening), at least 50 SPF protection is mandatory; however, our clinic's recommendation is medical sunscreens containing 130+ SPF that completely rule out the risk of blemishes.
  • Barrier Repair Schedule: Professional follow-up is provided by prescribing day by day which epithelizing and barrier-strengthening creams the patient should apply, for how long, and on which days after the procedure.

Lifestyle Factors Affecting Treatment Success

It should not be forgotten that the treatment we apply in our clinic gives a strong "repair instruction" to the skin. However, the quality and speed of this biological construction depend entirely on the person's own cellular healing capacity. For the collagen production triggered by the laser to reach its maximum level, the patient's drinking plenty of water, eating a diet rich in protein and vitamins, paying attention to sleep patterns, and providing uncompromising protection against the sun are vital factors that directly affect the ultimate success of the treatment.

Conclusion

By combining high-tech metal tube Fractional CO2 laser systems with modern dermatological approaches (such as subcision), it is possible to reconstruct tissue architecture from scratch at the cellular level. These procedures, which combine the skin's natural repair mechanisms with the latest technology, make achieving a smooth, healthy, and strong skin structure predictable, safe, and permanent at the cellular level.
